Doctor–patient communication is a crucial component in any therapeutic encounter. Physicians use words to formulate diagnoses and prognoses, to disclose the risks and benefits of medical interventions, and to explain why, how, and when a therapy will be administered to a patient. Likewise, patients communicate to describe their symptoms, to make sense of their conditions, to report side effects, to explore other therapeutic options, and to share their feelings. Throughout the history of medicine, the ethics of the doctor–patient communication has (...) been traditionally grounded on considerations of paternalistic beneficence and nonmaleficence. This article posits that in Foucault’s later work we find not a forgetting of literature, but a reformulation of the role that literature had come to occupy in his work, and that, in this later work, there is a trace of the texts on literary thought that he wrote from the 1960s to the mid-1970s. Drawing upon Foucault’s later work, I reconsider the key question: What relationship does literature, as literature, have with the political dimension of discourses? We show that certain classes of modules have universal models with respect to pure embeddings: Let R be a ring, T a first‐order theory with an infinite model extending the theory of R‐modules and (where ⩽pp stands for “pure submodule”). We show that limit models with chains of long cofinality are pure‐injective and we characterize limit models with chains of countable cofinality. This study explores how distributing the controls of a video game among multiple players affects the sociality and engagement experienced in game play. A video game was developed in which the distribution of game controls among the players could be varied, thereby affecting the abilities of the individual players to control the game. An experiment was set up in which eight groups of three players were asked to play the video game while the distribution of the game controls was increased (...) in three steps. After each playing session, the players’ experiences of sociality and engagement were assessed using questionnaires. The results showed that distributing game control among the players increased the level of experienced sociality and reduced the level of experienced control. The game in which the controls were partly distributed led to the highest levels of experienced engagement, because the game allowed social play while still giving the players a sense of autonomy. The implications for interaction design are discussed. (shrink)

ObjectiveHead and neck cancer treatments are known to significantly affect functionality and appearance, leading to an increased risk for body image disturbances. Yet, few longitudinal studies exist to examine body image in these patients. Based on a conceptual model, the current study aimed to determine, in patients newly diagnosed with HNC: the prevalence, level, and course of body image concerns; correlates of upon cancer diagnosis body image concerns; predictors of immediate post-treatment body image concerns; and association between body image concerns (...) and levels of anxiety, depression, suicidal ideation, support, and alcohol and drug misuse.MethodsTwo hundred and twenty-three, newly diagnosed with a primary HNC were assessed using structured clinical interviews and psychometric measures at three, and 6 months after diagnosis. Primary outcome was 3-month, as it was most salient to body image disturbance. Multiple linear regression analyses were conducted on the potential body image predictors, based on the model.ResultsSixty-eight percent of patients with HNC presented some level of body image concerns. Body image concerns at baseline and post-treatment were significantly related and significantly increased from pre- to post-treatment. Immediately post-treatment, 89% presented some level of body image concerns. Correlates of body image concerns in patients with HNC at baseline included: physical symptom burden, difficulties with communication and eating, coping with the cancer diagnosis using denial, suicidal ideation, and having had a past anxiety diagnosis. When controlling for sociodemographic and medical variables, body image concerns in patients with HNC in the immediate post-treatment were predicted by: baseline body image, physical symptom burden, and neuroticism.ConclusionThis longitudinal study helps identify patients more susceptible to experience body image disturbance following head and neck cancer. Clinicians ought to pay special attention to body image concerns upon cancer diagnosis, physical symptom burden, and neuroticism, and may want to target these factors in future preventive interventions. In this article we present the bases for a computational theory of the cognitive processes underlying human communication. The core of the article is devoted to the analysis of the phases in which the process of comprehension of a communicative act can be logically divided: (1) literal meaning, where the reconstruction of the mental states literally expressed by the actor takes place: (2) speaker's meaning, where the partner reconstructs the communicative intentions of the actor; (3) communicative effect, where the partner (...) possibly modifies his own beliefs and intentions; (4) reaction, where the intentions for the generation of the response are produced; and (5) response, where an overt response is constructed. The model appears to be compatible with relevant facts about human behavior. Our hypothesis is that, through communication, an actor tries to exploit the motivational structures of a partner so that the desired goal is generated. A second point is that social behavior requires that cooperation be maintained at some level. In the case of communication, cooperation is, in general, pursued even when the partner does not adhere to the actor's goals, and therefore no cooperation occurs at the behavioral level. This important distinction is reflected in the two kinds of game we introduce to account for communication. The main concept implied in communication is that two agents overtly reach a situation of shared mental states. Our model deals with sharedness through two primitives: shared beliefs and communicative intentions. (shrink)

The effects of striatal dopamine (DA) on behavior have been widely investigated over the past decades, with “phasic” burst firings considered as the key expression of a reward prediction error responsible for reinforcement learning. Less well studied is “tonic” DA, where putative functions include the idea that it is a regulator of vigor, incentive salience, disposition to exert an effort and a modulator of approach strategies. We present a model combining tonic and phasic DA to show how different outflows triggered (...) by either intrinsically or extrinsically motivating stimuli dynamically affect the basal ganglia by impacting on a selection process this system performs on its cortical input. The model, which has been tested on the simulated humanoid robot iCub interacting with a mechatronic board, shows the putative functions ascribed to DA emerging from the combination of a standard computational mechanism coupled to a differential sensitivity to the presence of DA across the striatum. In this paper it is argued that much can be gained for the analysis and evaluation of arguing when fallacies are not, or not only, conceived of as flawed premise–conclusion complexes but rather as argumentative moves which distort harmfully an interaction aiming at resolving communication problems argumentatively. Starting from Normative Pragmatics and the pragma-dialectical concept of fallacy, a case study is presented to illustrate a fallacy which is termed the 'revelation argument' because it is characterized by an interactor's revealing her (...) thoughts and/or emotions to the addressees and claiming that these would have justificatory or refutatory potential with respect to the problem discussed. Although the revelation argument may not be a paradigm case of resolution- hindering moves, it is an extreme case of flawed reasoning that illustrates plainly the advantages of a communicational perspective on arguing and fallacies. In two well-known passages from Paul Ricœur’s work (Ricœur 1990b, 187; 2006, 260), the author proposes approaching memorial writing of the Holocaust not necessarily in the same terms as historiography. On the basis of these passages, the aim of this article is to further explore Ricœur’s intuition by suggesting a comparison with the prose of a contemporary author who intentionally seeks to create a hybrid between history and fiction: W. G. Sebald. Although Sebald never considered himself a novelist, his writing (...) explicitly addresses the challenge of representing trauma, particularly in relation to the Holocaust. This article applies Ricœur’s insights on the function of the productive imagination and the effect on the reader to Sebald’s fragmentary style of writing. My thesis, derived from this application, insists on viewing memory in the face of horror and trauma not as something merely representational, but as a dynamic process we actively engage in, highlighting Ricœur’s emphasis on the role of reader in shaping our understanding of the past. Corporate psychopathy thrives perhaps as the most significant threat to ethical corporate behaviour around the world. We argue that Human Resources Management professionals should formulate strategic solutions metaphorically by balancing what strategic military planners famously call ‘Search and Destroy’ and ‘Hearts and Minds’ counter-terrorist strategy. We argue that these military metaphors offer creative inspiration to help academics and practitioners theorise CP in richer, more reflective and more balanced and complementary ways. An appreciation of both metaphors is likely to favour the (...) use of hybrid strategies comprising SD and HM elements, which may provide the best HRM solutions to CP for the same reasons as the military now considers parallel hybrid solutions optimal for combating terrorist and guerrilla insurgency. Experimental studies have suggested that variables such as aptness or conventionality are predictors of people’s preference for expressing a particular topic–vehicle pair as either a metaphor or a simile. In the present study, we investigated if such variables would also be predictive within a more naturalistic context, where other variables, such as the intention to include an explanation, may also influence people’s decision. Specifically, we investigated the production of metaphor and simile expressions on the Internet via the Google search engine (...) and checked for accompanying explanations, as well as the properties they expressed, to examine whether ratings such as aptness, conventionality, as well as participants’ own stated preference or the intention to produce an explanation, would predict which topic–vehicle pairs appeared more often as metaphor. We found that participants’ stated preference predicted metaphor dominance on the Internet, and that apt topic–vehicles occurred more often as metaphors. The explanations collected, however, occurred 82% of the time after similes, and familiar expressions were the most explained. Finally, comparing the properties expressed in these explanations to normed property lists, we found that simile explanations typically expressed a novel conception of the topic–vehicle relationship. Therefore, we found that Internet posters use metaphors to convey an apt relationship, as found in previous laboratory studies, but prefer using a simile frame when they want to express a relationship that readers will find novel. With the purpose of introducing a useful tool for researches concerning foundations of quantum mechanics and applications to quantum technologies, here we address three quantumness quantifiers for bipartite optical systems: one is based on sub-shot-noise correlations, one is related to antibunching and one springs from entanglement determination. The specific cases of parametric downconversion seeded by thermal, coherent and squeezed states are discussed in detail.

Hempel’s account of thought experiments has been discussed only by a very few authors and, for the most part, with rather cursory remarks. Its importance, however, is not only historical, but also systematic theoretical, because it involves the distinction between discovery and justification, a main pillar of neopositivistic philosophy of science. Hempel raised the question whether thought experiments constitute a methodological component of scientific research or, on the contrary, are merely a heuristic-psychological device for obtaining and/or transmitting new ideas. While (...) conceding a few exceptions in the natural sciences, he argued that thought experiments always have a heuristic character in the social sciences. There is however a fundamental tension in Hempel’s conception of thought experiments, between the thesis of methodological monism and the neopositivistic dichotomy discovery/justification. On the one hand, on the basis of the unity of scientific method, Hempel admits a difference only in degree between the natural and the human sciences, but on the other hand, he draws a principled distinction between thought experiments of the human sciences (which have only a greater or lesser heuristic value) and those of the natural sciences (which may have also a cognitive-justificatory value). If one assumes the unity of method in the minimal sense in which no scientific knowledge can renounce intersubjective controllability, this tension can be removed either by rejecting the discovery/justification dichotomy or by interpreting it differently. Here, following the second path, two senses of the dichotomy are distinguished, one of which must be accepted, while the other rejected. This removes the internal tension in Hempel’s conception of thought experiments and suggests the thesis that any plausible thought experiment, both in the natural and the human sciences, must already contain some justification, implicit or explicit, of the theoretical hypotheses that they formulate. Human adults incline toward moral objectivism but may approach things more relativistically if different cultures are involved. In this study, 4-, 6-, and 9-year-old children (N = 136) witnessed two parties who disagreed about moral matters: a normative judge (e.g., judging that it is wrong to do X) and an antinormative judge (e.g., judging that it is okay to do X). We assessed children’s metaethical judgment, that is, whether they judged that only one party (objectivism) or both parties (relativism) could (...) be right. We found that 9-year-olds, but not younger children, were more likely to judge that both parties could be right when a normative ingroup judge disagreed with an antinormative extraterrestrial judge (with different preferences and background) than when the antinormative judge was another ingroup individual. This effect was not found in a comparison case where parties disagreed about the possibility of different physical laws. These findings suggest that although young children often exhibit moral objectivism, by early school age they begin to temper their objectivism with culturally relative metaethical judgments. (shrink)
